An image that Geri finds scary or disturbing could trigger a responsive behaviour.
htmlText_66E86192_76D5_937D_4164_3CBB8988F802.html =
CORRECT
Geri may not recognize themself in the mirror and become startled or afraid believing someone else is in the room which could trigger responsive behaviour.
htmlText_6537BB5D_76F2_B7F4_41D7_FAF56566AF08.html =
CORRECT
The process of assisting Geri with showering may be triggering. Geri may not trust a stranger to remove their clothing and bathe them, and/or may dislike being cold when undressed.

NOT THE BEST STRATEGY
Geri may not have the capacity to understand or exercise good judgment when it comes to hygiene practices.
INSTEAD TRY...
Offering an incentive to motivate Geri to shower. By providing Geri with something they find particularly rewarding will increase the likelihood of the desired action, in this case, the shower.

Geri may confuse the robe with an actual person which could cause distress.
htmlText_5265BC35_73BD_A1BE_41D2_CE3A1101C2E6.html =
CORRECT
Geri may not be able to express that the room is too hot or cold. A temperature that Geri finds uncomfortable could trigger a responsive behaviour.
htmlText_73D2B1A2_6CAC_A254_41CB_E53DBC124C2F.html =
CORRECT
Geri may perceive their privacy as being infringed upon, which could trigger behaviours. Room dividers also do not adequately control for noise; thus, unexpected loud noises from roommates can trigger behaviours.
htmlText_538DA08E_7364_6154_41C3_254857010F61.html =
CORRECT
Individuals may perceive the mat as something else (e.g., a hole in the ground), which could trigger behaviours. The mat may also pose a tripping hazard.
htmlText_77599036_6C95_198E_41DA_F3741CC8AE1E.html =
CORRECT
Loud noises or disturbing content could trigger a responsive behaviour. The 2 dimensional images on the TV could also cause disorientation and confusion, or difficulty differentiating between real and imagined scenarios.
htmlText_7952E357_6CA4_6681_41BC_D082428E3E6C.html =
CORRECT
The door may be a trigger for Geri, as exit seeking is a common responsive behaviour. Having multiple doors could potentially be confusing and disorienting.
htmlText_5ED64882_74A4_A117_41D8_53F9F88CF966.html =
CORRECT
The process of toileting may be triggering, especially if Geri has difficulty with sit/stand transitioning, or is confused about how to use the toilet.
htmlText_716607CC_6CA5_ADFD_41C4_CC54F4EA2D2E.html =
CORRECT
Though this is not an obvious trigger, the low seating and crowded space poses a barrier to movement and function which may trigger a responsive behaviour, especially in individuals with gait aids (e.g., walkers) or wheelchairs. The furniture and equipment could also pose a tripping hazard in individuals with poor visual spatial abilities.

GOOD CHOICE
Applying safety locking door handles would make it more difficult to exit and may deter Geri from leaving.
It’s also a good idea to clearly label the inner doors (such as “closet” and “bathroom”) to minimize confusion and disorientation, and limit accidental exit. You could also disguise exit doors with painted images such as bookshelves or a natural landscape which can minimize distress and deter exit.

Having a TV remote nearby allows volume or content to be easily changed (by the person or their caregiver, depending on abilities) to suit individual preferences. This would also minimize feelings of helplessness, lack of control, and frustration, which are also common behavioural triggers.
Since Geri is in a shared environment where muting the volume is not always possible, noise cancelling headphones can reduce unwanted stimulation for Geri while still providing appropriate levels of stimulation for other residents.
As a general safety precaution, it is a good idea to secure heavy objects such as a TV. This avoids them being thrown if someone were upset.

Individuals with cognitive impairment benefit from familiar environments and routine. Geri will likely do best in their current space, which has become familiar. Moreover, there are several considerations to be made prior to implementing room changes. For example, there may not be any private rooms available, or Geri’s funding package may not cover a private room. There are other ways to make Geri comfortable without switching rooms.
INSTEAD TRY...
Ensuring that the room divider is properly drawn at all times and that Geri’s room is not visible to other residents. This will reduce the risk of Geri’s room being visible to his roommate, which will minimize the risk of Geri feeling that his private space is being invaded.
Since Geri is in a shared environment where the needs and preferences of other residents must also be considered, ridding the environment of noise may not always be possible, and drawing the room divider may not be sufficient. Consider having Geri use noise cancelling headphones to reduce unwanted noise, while still respecting roommates’ freedom to engage in desired levels of stimulation (e.g., TV, phone calls).

Having a TV remote nearby allows volume or content to be easily changed (by the person or their caregiver, depending on abilities) to suit individual preferences. This would also minimize feelings of helplessness, lack of control, and frustration, which are also common behavioural triggers.

As a general safety precaution, it is a good idea to secure heavy objects such as a TV. This avoids them being thrown if someone were upset.

If Geri is initially resistive, you could walk away and re-approach at a later time to avoid escalating behaviour in the moment. Make sure the bathroom is heated, the water is at a warm temperature, and the towel/robe is warm (use a towel warmer where possible). Using a hand-held shower head may also facilitate cleaning of hard to reach areas, or quickly cleaning specific areas in isolation.
Encouraging independence can make Geri feel more in control which would reduce the likelihood of behaviours occurring. Have Geri participate in showering based on their capabilities using verbal prompts and gestures. For example, give them the soapy washcloth to wash under their arms.
If a full shower is not possible due to extreme agitation, provide Geri with a bed bath, uncovering one body part at a time to limit shame and embarrassment.
Always make sure that the shower space is functional for Geri’s level of ambulation, and monitor for safety.

Always make sure that the shower space is functional for Geri’s level of ambulation, and monitor for safety.

htmlText_DE4E11D2_C927_4BAD_41E2_F6250683CD4D.html = Summary of triggers and mitigation strategies
Loud TV - Loud noises or disturbing content could trigger a responsive behaviour. The 2 dimensional images on the TV could also cause disorientation and confusion, or difficulty differentiating between real and imagined scenarios.
Have a TV remote nearby allows volume or content to be easily changed (by the person or their caregiver, depending on abilities) to suit individual preferences.
Door - The door may be a trigger for Geri, as exit seeking is a common responsive behaviour. Having multiple doors could potentially be confusing and disorienting.
Room temperature - Geri may not be able to express that the room is too hot or cold. A temperature that Geri finds uncomfortable could trigger a responsive behaviour.
Layers can be easily removed or applied as needed depending on preference. It’s a good idea to have a blanket nearby to provide extra warmth and comfort when necessary.
Bright lights - Although older adults like Geri often need more light due to changes in vision (e.g. cataracts, macular degeneration, glaucoma), bright lights can be triggering for some individuals.
Wherever possible, use natural lighting to avoid behavioural responses. Natural light also helps to induce positive mood. You could also install a dimmer switch for the lights. Changing the amount of light exposure is a useful way to regulate behaviour. For example, dim lighting can be used to calm and relax, while bright lighting can be used to arouse and awaken. Lighting preferences are very individual, and the amount of light should be tailored to individual needs.
Clutter and furniture - Though this is not an obvious trigger, the low seating and crowded space poses a barrier to movement and function which may trigger a responsive behaviour, especially in individuals with gait aids (e.g., walkers) or wheelchairs. The furniture and equipment could also pose a tripping hazard in individuals with poor visual spatial abilities.
Removing or minimizing barriers to movement can help minimize distress and safety risks. For example, leave enough room to easily move around chairs and tables.
Since Geri’s needs fluctuate throughout the day, there may be times when equipment is needed and it is helpful to have it close by. At other times of day the clutter may pose safety hazards for Geri and other residents if not carefully positioned.
Floor Mat - Individuals may perceive the mat as something else (i.e., a hole in the ground), which could trigger behaviours. The mat may also pose a tripping hazard.
Removing floor mats will reduce misperception and tripping hazards, especially for individuals with poor visual spatial or navigation abilities.
Disturbing imagery - An image that Geri finds scary or disturbing could trigger a responsive behaviour.
Hanging more pleasant images will allow for appropriate stimulation, while reducing the risk of misperception or confusion that may trigger a responsive behaviour. Try hanging neutral images with natural elements (e.g., landscapes) and calming tones (e.g., blue, green, earth tones).
Loud phone - An unexpected loud noise can trigger a responsive behaviour.
Minimizing the noise will also minimize the startle response and accompanying behaviour. You could also select a more pleasant ringtone to ensure that the ringing phone is less bothersome.
Privacy divider - Geri may perceive their privacy as being infringed upon, which could trigger behaviours. Room dividers also do not adequately control for noise; thus, unexpected loud noises from roommates can trigger behaviours.
Properly drawing the room divider will reduce the risk of Geri’s room being visible to his roommate, which will minimize the risk of Geri feeling that his private space is being invaded.
Mirror- Geri may not recognize themself in the mirror and become startled or afraid believing someone else is in the room which could trigger responsive behaviour.
Removing the mirror would prevent potential unnecessary distress. If it is not possible to safely remove the mirror, you could cover it with a sheet, towel, or brown paper until such time that it can be removed.
Hanging robe or clothing - Geri may confuse the robe with an actual person which could cause distress.
Folding the robe would remove the potential for misperception, and still provide an easily accessible covering post-shower.
Shower - The process of assisting Geri with showering may be triggering. Geri may not trust a stranger to remove their clothing and bathe them, and/or may dislike being cold when undressed.
Providing Geri with something they find particularly rewarding will increase the likelihood of the desired action, in this case, the shower.
There are several other things you could do.
If a full shower is not possible due to extreme agitation, provide Geri with a bed bath, uncovering one body part and explain in simple language what you are doing.
Always make sure that the shower space is functional for Geri’s level of ambulation, and monitor for safety.
Toilet - The process of toileting may be triggering, especially if Geri has difficulty with sit/stand transitioning, or is confused about how to use the toilet.
Depending on Geri’s functional needs, they may require a raised toilet seat and/or handrails. They may also need assistance with donning/doffing pants, wiping and hand washing. Depending on their level of cognition, they may also require orientation to the toilet itself.

NOT THE BEST STRATEGY
Covering the artwork may cause confusion.
INSTEAD TRY...
Rather, hang more pleasant images instead. Hanging more pleasant images will allow for appropriate stimulation, while reducing the risk of misperception or confusion that may trigger a responsive behaviour. Try hanging neutral images with natural elements (e.g., landscapes) and calming tones (e.g., blue, green, earth tones).

NOT THE BEST STRATEGY
Not allowing Geri to watch TV puts unnecessary restrictions on their day to day activities. It may also cause frustration and/or conflict for Geri and the other LTC residents to have the TV present but not be allowed to engage with it. This could escalate to aggressive behaviour, which we want to avoid.
INSTEAD TRY...
Rather, have a TV remote within reach so volume or content to be easily changed (by the person or their caregiver, depending on abilities) to suit individual preferences.

Older adults need exposure to light for proper visual function, and overall stimulation.
INSTEAD TRY...
Rather, wherever possible, use natural lighting to avoid behavioural responses. Natural light also helps to induce positive mood.
In private rooms, you could also consider installing a dimmer switch for the lights. Changing the amount of light exposure is a useful way to regulate behaviour. For example, dim lighting can be used to calm and relax, while bright lighting can be used to arouse and awaken. Lighting preferences are very individual, and the amount of light should be tailored to individual needs.

Replacing the floor mat with one that is the same colour may prevent misperception of the mat, but it actually increases the tripping hazard by making it more difficult to see.
INSTEAD TRY...
Removing floor mats. This will reduce misperception and tripping hazards, especially for individuals with poor visual spatial or navigation abilities.

While restraints may be appropriate in some severe cases, this would not be an appropriate use of restraints.
INSTEAD TRY...
Placing safety locking door handles on all the doors to make it more difficult to exit and deter Geri from leaving.
It’s also a good idea to clearly label the inner doors (such as “closet” and “bathroom”) to minimize confusion and disorientation, and limit accidental exit. You could also disguise exit doors with painted images such as bookshelves or a natural landscape which can minimize distress and deter exit.

This module has audio.
Dementia causes changes to Geri’s brain. Apart from changes in memory, dementia also affects how Geri perceives and reacts to the environment. For example, Geri might not recognize familiar objects, or be able to interpret distances.
Geri can be easily confused, startled, or frightened because of their dementia, so they may respond with a behaviour that is inappropriate for the situation, which is known as a responsive behavior.
This module will take you through a tour of Geri’s long-term care home.
As you move through Geri’s home, you will be looking for things in the environment that may trigger responsive behaviours.
Click next to continue.
